1.Przedmiot zamówienia: dostawa leków do programu lekowego Modulatory CFTR do Samodzielnego Zespołu Publicznych Zakładów Opieki Zdrowotnej w Dziekanowie Leśnym. 2.Szczegółowy opis przedmiotu zamówienia zawierają niniejsza SWZ oraz: 1)załącznik nr 1 – formularz oferty; 2)załącznik nr 2 – formularz asortymentowo-cenowy; 3)załącznik nr 3 – projekt umowy. 3.Zamawiający nie dopuszcza możliwości składania ofert częściowych. Przedmiotem zamówienia jest jedna substancja lecznicza i w związku z tym nie ma możliwości podziału zamówienia na części. 4.Miejsce realizacji zamówienia: Apteka SZPZOZ. 5.Zamawiający nie dopuszcza możliwości składania ofert wariantowych. 6.Wszystkie wymagania określone w SWZ i dokumentach wskazanych powyżej stanowią wymagania minimalne, a ich spełnienie jest obligatoryjne. Niespełnienie ww. wymagań minimalnych będzie skutkować odrzuceniem oferty jako niezgodnej z warunkami zamówienia na podstawie art. 226 ust. 1 pkt 5 ustawy Pzp. 7.Zamawiający nie przewiduje udzielenia zamówień, o których mowa w art. 214 ust. 1 pkt 8 ustawy Pzp. 8.Zamawiający nie przewiduje skorzystania z prawa opcji. 9.Zamawiający dopuszcza składanie ofert równoważnych. Za ofertę równoważną uznaje się lek o takiej samej nazwie międzynarodowej i dawce oraz drodze podania. 10.Oznakowanie dostarczonych wyrobów, instrukcje użytkowania, opis wyrobu, etykiety (oryginalne) muszą być sporządzone w języku polskim. 11.Zgodnie z art. 138 ust. 2 pkt 2 ustawy Pzp, Zamawiający wyznacza termin składania ofert krótszy niż określony w art. 138 ust. 1 ustawy Pzp.
