1. Przedmiotem zamówienia jest zadanie inwestycyjne w formule „zaprojektuj i wybuduj” pn. „Poprawa dostępu do świadczeń opieki psychiatrycznej przez rozbudowę oraz dostosowanie infrastruktury budowlanej i technicznej wraz z wyposażeniem dla Centrum Zdrowia Psychicznego w SPS ZOZ w Lęborku”. Inwestycja została podzielona na 3 zadania:  ZADANIE 1: Nadbudowa i przebudowa Oddziału Psychiatrycznego (lokalizacja – Lębork, ul. J. Węgrzynowicza 13, obr. 7, dz. nr 243, budynek główny Szpitala nr ewidencji geodezyjnej 1483, 1486, 1660);  ZADANIE 2: Rozbudowa, nadbudowa i przebudowa Oddziału Dziennego Psychiatrycznego (lokalizacja – Lębork, ul. Węgrzynowicza 13, obr. 7, dz. nr 243, budynek Działu Fizjoterapii, nr ewidencji geodezyjnej 1663);  ZADANIE 3: Rozbudowa, nadbudowa i przebudowa Poradni Zdrowia Psychicznego z Punktem Zgłoszeniowo-Koordynacyjnym oraz Zespołami Leczenia Środowiskowego Centrum Zdrowia Psychicznego (lokalizacja – Lębork, ul. S. Żeromskiego 9-10, obr. 11, dz. nr 149/26). Szczegółowy opis przedmiotu zamówienia stanowi Program Funkcjonalno-Użytkowy stanowiący Załącznik nr 1 do SWZ.[E8]
