1. Przedmiotem zamówienia jest zakup i dostawę sprzętu jednorazowego do zabiegów ablacji. 2. Przedmiot zamówienia podzielony został na 5 części (5 pakiety). Pakiet nr 1. Sprzęt jednorazowy do zabiegów ablacji metodą elektroporacji połączonej z mapowaniem 2 D Pakiet nr 2. Stymulatory bezelektrodowe jednojamowe komorowe lub przedsionkowe Pakiet nr 3. Dzierżawa kamizelek defibrylacyjnych 200 jednostek Pakiet nr 4. Folie sterylne do systemu Zero Gravity Pakiet nr 5. Elektroda defibrylacyjna z małą średnicą certyfikowania do LBBAP Szczegółowy opis przedmiotu zamówienia stanowi Załącznik Nr 6 do SWZ - (Opis przedmiotu zamówienia/ Formularze parametrów technicznych). W każdym przypadku, jeżeli w SWZ nie ma wskazania do której części zamówienia odnosi się dany zapis, należy przyjąć, iż zapis dotyczy obydwu z części zamówienia. a) Wspólny Słownik Zamówień CPV: Główny kod CPV: 33182200-1. Urządzenia do stymulacji pracy serca 33100000-1. Urządzenia medyczne 3. Oferty można składać w odniesieniu do: wszystkich części. Zamawiający nie określa maksymalnej liczby części zamówienia, na które może zostać udzielone zamówienie jednemu Wykonawcy. Zamawiający zastrzega jednak składanie ofert przez Wykonawcę na całość asortymentu w danym pakiecie. 4. Bezwzględnie wymagane jest, by zaoferowane wyroby medyczne, stanowiące przedmiot zamówienia, spełniały wymagania określone w ustawie z dnia 7 kwietnia 2022 r. o wyrobach medycznych (t.j.
