Przedmiotem zamówienia niniejszego postępowania przetargowego są dostawy produktów leczniczych, w tym w ramach programów lekowych zwanych dalej „towarem” zgodnie z rodzajem asortymentu i ilościami określonymi w „Formularzu asortymentowo-cenowym” stanowiącym załącznik nr 2 do SWZ i zgodnie z „Formularzem Oferty”, stanowiącym załącznik nr 1 do SWZ. Zamówienie jest podzielone na 8 części (pakietów). W Formularzu asortymentowo-cenowym, w odpowiednich kolumnach poszczególnych części, są wskazane ilości, jakie zostaną zamówione przez Samodzielny Publiczny Zakład Opieki Zdrowotnej Uniwersytecki Szpital Kliniczny nr 2 Uniwersytetu Medycznego w Łodzi oraz Samodzielny Publiczny Zakład Opieki Zdrowotnej Uniwersytecki Szpital Kliniczny nr 1 im. N. Barlickiego Uniwersytetu Medycznego w Łodzi. Każdy z Zamawiających podpisuje odrębną umowę w sprawie udzielenia zamówienia w zakresie ilości określonych w Formularzu asortymentowo-cenowym dla danego szpitala.[E7]
