1. Działając w imieniu i na rzecz Ministerstwa Zdrowia, na podstawie udzielonego pełnomocnictwa, Centrum e-Zdrowia z siedzibą w Warszawie (kod pocztowy 00-184) ul. Stanisława Dubois 5A ogłasza postępowanie na zamówienie publiczne, którego przedmiotem jest świadczenie Usługi Serwisu Urządzeń wraz z Oprogramowaniem będących w posiadaniu Zamawiającego, tj. Ministerstwa Zdrowia w Warszawie przy ul. Miodowej 15. 2. Zamówienie zostało podzielone na sześć części: Część I – urządzenia Actidata Części II - urządzenia Cisco Części III - urządzenia Dell Części IV - urządzenia Fortinet Części V - urządzenia HPE Części VI - urządzenia IBM 3.Szczegółowy opis przedmiotu zamówienia stanowi Załącznik nr 1 do SWZ - Opis przedmiotu zamówienia oraz Załącznik nr 2 do SWZ - Projektowane postanowienia umowy. 4.Wykonawca zobowiązany jest zrealizować zamówienie na zasadach i warunkach opisanych w Projektowanych postanowieniach umowy stanowiących Załącznik nr 2 do SWZ.[E8]
