Hankinnan kohteena on Kansaneläkelaitoksen (Kelan) henkilöasiakkaille järjestettävä kuntoutuspalvelu, Kuntoutustukea saavan nuoren ammatillinen valmennus, ja sen kehittäminen. Tämä tarjouspyyntö koskee Etelä-Suomen alueelle kilpailutettavaa palvelua. Hankittava palvelu on osa Kuntoutustukea saavien kuntoutuksen kehittämisprojektia, joka toteutetaan vuosina 2025-2028. Projektissa luodaan uusi yksilöllinen ammatillinen kuntoutuspalvelu asiakkaille, jotka saavat kuntoutustukea ainoastaan Kelasta. Kuntoutustukea saavan nuoren ammatillisen valmennuksen kohderyhmänä on 18-29 -vuotias asiakas, jonka sairaus tai vamma on aiheuttanut tai sen arvioidaan aiheuttavan lähivuosina asiakkaan työ- tai opiskelukyvyn ja ansiomahdollisuuksien olennaisen heikentymisen Kelan kuntoutuslain 6 § mukaisesti (Kansaneläkelaitoksen kuntoutusetuuksista ja kuntoutusrahaetuuksista annettu laki 566/2005). Asiakkaalle on myönnetty Kelan maksama kuntoutustuki, jota asiakas on saanut alle 2 vuotta. Asiakkaan työ- ja toimintakyvyn on arvioitu palautuvan asianmukaisella hoidolla ja/tai kuntoutuksella. Valmennuksen tavoitteena on aktivoida asiakasta siirtymään kuntoutustuelta opintoihin, työelämään tai muihin kuntoutuspalveluihin. Kuntoutustukea saavan nuoren ammatillinen valmennus toteutetaan yksilökuntoutuksena. Valmennuksen toteuttaa valmentaja ja se sisältää sekä kasvokkaista kuntoutusta että etäkuntoutusta. Palvelu sisältää tapaamisia, joista osa voidaan toteuttaa kuvapuheluina, lisäksi palveluun sisältyy verkkokuntoutusta sekä verkostotyötä.
