Perusterveydenhuollon hoidon jatkuvuuden ammatinharjoittajamallin hankinta[E14]

Hankinnan kohteena on yleislääkäripalveluiden hankinta ammatinharjoittajamallilla toteutettuna Pirkanmaan hyvinvointialueelle. Hankinta sisältää yhteensä neljän (4) lääkärin ostopalvelun Hatanpään sosiaali- ja terveysaseman ja Tammelakeskuksen sosiaali- ja terveysaseman alueiden omapotilaiden hoidon toteuttamiseksi. Ammatinharjoittajamallilla tuotetaan yleislääkäripalvelut kokonaisvaltaisesti hoidon jatkuvuusmallin kriteerien mukaisille omapotilaille, jotka asuvat yrittäjälääkärille osoitetun sote-aseman alueella. Yrittäjälääkäri toimii omalääkärinä, joka on hoitovastuussa omapotilaistaan. Omalääkärin työparina toimii omahoitaja. Yrittäjälääkäri toimii yhteistyössä tilaajan henkilöstön kanssa osana sote-aseman moniammatillista toimintaympäristöä. Palvelussa keskeistä ovat hoidon saatavuuden sekä hoidon jatkuvuuden parantaminen. Palvelu tuotetaan joko edellä mainituilla Tilaajan sosiaali- ja terveysasemilla tai Palveluntuottajan järjestämissä toimitiloissa. Toimitiloja koskevat sijaintivaatimukset on määritetty hankintasopimuksen liitteessä 2 Palvelukuvaus. Sopimuskausi alkaa 1.2.2027 ja on voimassa toistaiseksi. Hankinnan ennakoitu arvonlisäveroton vuosiarvo sitoumuksetta on noin 875 000 euroa. Palveluntuottajien valinta on kuvattu tarkemmin tarjouspyynnön kohdassa ”Päätöksenteon perusteet”. Hankinnan kohde ja vähimmäisvaatimukset on kuvattu tarjouspyynnössä ja tarjouspyynnön liitteissä.[E8]
